** 100% Secure & Safe Payments ** By submitting this form, I hereby acknowledge and agree that participation in martial arts classes and activities at Master Bill's Karate (hereinafter referred to as "the School") involves inherent risks and dangers. I understand that while the School takes all reasonable precautions to ensure the safety of its participants, injuries can still occur. In consideration of being allowed to participate in the martial arts classes and activities provided by the School, I hereby voluntarily assume all risks associated with my participation, including, but not limited to, the risk of bodily injury, disability, or loss of life. I understand and accept that martial arts training involves physical contact, strenuous physical activity, and the use of potentially dangerous equipment. I acknowledge that the instructors and staff at Master Bill's Karate are trained professionals who prioritize safety and adhere to industry-standard practices. They will take all reasonable measures to minimize the risks associated with martial arts training. However, I understand that accidents and injuries can still occur despite these precautions. In consideration of being permitted to participate in the martial arts classes and activities, I hereby release, discharge, and hold harmless Master Bill's Karate, its instructors, staff, and any affiliated parties from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in martial arts classes and activities at the School. I understand and agree that this waiver and release of liability is binding upon myself, my heirs, executors, administrators, and assigns. I hereby certify that the student I am enrolling, either myself or my family member, is physically fit to participate in martial arts training and has no medical conditions that would prevent their safe participation. I have carefully read and understood this waiver and release of liability. I am aware that by submitting this form, I am waiving certain legal rights, including the right to sue Master Bill's Karate for any injuries or damages that may occur during my participation. Membership is on a pre-paid basis, and all sales are considered final. Refunds are not offered. If you should wish to cancel your membership at some point, please contact the school and request a cancellation.
